This questionnaire is to collect feedback from participants of the Project Management Workshop held on 26 January 2006. Access is open to all participants immediately after the Workshop to 2359 on 28 January 2006, or roughly 2 days later. Results of the survey will be used to evaluate the effectiveness of the workshop in achieving its objectives.
 
All workshop participants can gain access to this questionnaire by using the generic password assigned to the whole class, but in order to guarantee valid and non-repetitive returns, customized random passwords have been distributed to individual participants. Such passwords must be entered when answering this questionnaire, otherwise the return will be deemed invalid and will not be used. Because nobody except the owner knows which password is assigned to which participant, individual identity will be fully protected.
